證書發出者描述 (從左至右，證書正面) : 「EDUCATION DEPARTMENT, HONG KONG. / LOCAL EXAMINATION SYNDICATE / Hong Kong School Certificate Examination. / ADMISSION CARD / Admit Candidate Number 3552 / Name LU KWONG FAI / School KOWLOON ENGLISH SCHOOL / Candidate's Signature Lu Kwong Fai / Date February 20, 1960 / I hereby certify that the attached photograph is a true likeness of LU KWONG FAI / Signature (across the photograph) of the Head of the School or College presenting the candidate.」 ; 證書背面描述 : 「N.B.－This card must be placed on the desk in fornt of candidate throughout the examination. / NOTICE TO CANDIDATES. / 1. The doors of the Examination-Room will be closed half-an-hour after each paper begins ... Candidates taking Chemistry will be permitted, if they desire it, to use a stencil for making drawings of apparatus.」
米黃色紙張，黑墨印刷，黑色原子筆標示。證書上方印有英國政府所使用的盾徽 (缺少代表英格蘭的獅子和代表蘇格蘭的獨角獸)。
香港中學畢業會考由教育司署於1946年開始舉辦。 到1952年， 政府開始為中文中學的學生舉辦香港中文中學高中畢業會考 (HKCSCE)，此考試有別於香港中學畢業會考。其後，香港中學畢業會考於1961年改名為香港英文中學畢業會考 (HKESCE)。1974年，香港中文中學會考與香港英文中學會考合併為香港中學會考 (HKCEE)。 而隨著三年制高中學制於2009年9月開始實施， 香港中學會考將於2012年被香港中學文憑考試 (HKDSE)取代。
此准考證背面印有考生需知，考生需將此證放在桌子上以便監考員核對身份。
